Accutane - negative experiences

Hello to everybody,

I want to share with you some negative experiences about Accutane. I have never taken Accutane, so I can only give you some experiences from people who had tried it in the past.
As I understood, in some cases Accutane caused heavy and irreversible damages, so I am asking myself, is it worth to risk one’s life like this?
Guys, I discourage you to follow this way – search for different therapies  recover from your acne. Believe me, I fully understand sometimes anguish can push you to opt for strong methods, but it is not always true that something strong is something right.
I have do Internet researches and I have found some negative experience to be share with you. 
I will be happy if I could receive your experiences or ideas back.
If you had negative situations, I will be glad to know them; I would like to share them with who suffered from the same problems and with who could suffer from one day if deciding to take Accutane. 
2.000 cases of depression, about a thousand of suicides and suicide attempts have been advised due to Accutane. 6.000 instances of psyche disorder have been considered.
These data are contained in a database of side effects  by the World Healthcare Organization and are related to the medicine produces by the Swiss multinational Roche. Pursuant to American authorities, these numbers should be multiply per hundred – because just one per cent of the side effects are  signaled. According to the reports, Accutane has caused / is causing a massacre. The drug has been a blockbuster for Roche for two decades – the licence expired in 2002 and nowadays many generic medicines can be found in the market. The biggest consumers are teenagers. According to Falò inquiries, a Ticino program, doctor and healthcare authorities are unaware of Accutane risks. But 152 side effects have been identified. The upsetting  results based on the medicine studies and even a case of suicide occurred during the Accutane test have always been hidden from Roche. From Ticino to Unites States, from Ireland to Switzerland victims, layers, scientists and Swissmedic are talking about Accutane. The big absentee is the Swiss multinational: Roche is not issuing statements about Accutane.

Bad experience with Accutane

I know how devastating it is to have bad skin. I've had horrible acne for 12 years now. I thought Accutane was going to be my miracle cure. I was scared because of all the crazy side effects, but I thought that NONE of them could be worse than having acne. Unfortunately, I was wrong. See, Accutane affects everybody differently, but I literally starting going mental on this drug. Only while on day 10 was I hallucinating and having fits of intense and disabling paranoia. I was so scared to leave my bed that I peed in the cup on my nightstand. I couldn't drive because I was afraid that someone was going to jump out from behind my seat and I had a reoccurring vision of a woman choking me that I would get flashbacks of from time to time. I began to become afraid of everything. I even wanted to work overtime at work because I was afraid to go home by myself. Never ever have I had psychological issues such as these ones and I never saw day 11 of Accutane. My doctor immediately took me off the drug and I am currently waiting for the drug to run its course through my body. Does this happen to everyone? Absolutely not. However, do your research because I wouldn't wish my experiences on anybody. Type Accutane and Hallucinations in the same search engine and see how many articles come up of cases such as mine. Look at the # of suicides and psychotic episodes of people taking Accutane. It is most definitely a miracle drug for acne, but in my experience--beauty was not worth my mental health. Horrible Experience on Accutane - Still Having Side Effects

I started taking Accutane last September to deal with my back acne. Two days into treatment I thought I had a bout of severe food poisoning. I was throwing up every hour all night. I stopped taking Accutane for a week to get better from my “food poisoning” which I no longer believe was food poisoning just my system rejecting the drug.

Anyways I went back on Accutane for the next few months having really dry lips, dry skin, dry hair (no hair loss), and mild headaches (these are the only symptoms I was warned of and they seemed pretty reasonable and normal). I suffered from constipation to the point where I saw some blood after going to the bathroom (was not warned of this by my dermatologist and when I told her about it she didn't think it could be related). I also had really dry eyes with a sharp pain developing in my left eye. In December my dosage got increased.

A week or two after the dosage increase I started having very vivid dreams and nightmares and waking up drenched in sweat. I started feeling sinus and head pressure similar to a bad sinus infection but I did not have a sinus infection (I was checked by a doctor). I started having darker urine that had a smell it never had before and in January I was rushed to the ER because my heart was pounding, I couldn't breathe, I had difficulty swallowing, and I couldn't stop shaking. At the ER I was diagnosed with a panic attack (something I have never had before in my life) and sent home. I had made mention that I was on Accutane but the doctor didn't say anything about it or that it could be the cause (the doctor didn't really seem to know what Accutane was).

While at home the body spasms and shaking, nausea, head pressure, and fatigue wouldn't go away and I assumed it was a bad virus. I stayed home from work all week in bed (not having any energy to get out of bed or appetite to eat). During this week I stopped taking Accutane to let my body recover from this “virus” and it did, well almost (At this point I still didn't even think Accutane could be the cause of all this, it sounds stupid reading all these symptoms back but I really didn't). I wasn't completely better but at least I had some of my strength back and could manage driving to work again. I was feeling better so I went back on the Accutane.

This is where I finally made the connection. Shortly after starting the treatment again I was feeling exactly the same way as I did when I was rushed to the ER in January. I called my dermatologist immediately and she said I should get off Accutane completely (but did not offer any advise of how to treat the symptoms or who to see). I stopped the Accutane treatment immediately.

That was almost six weeks ago. Most of the symptoms have lessened since I stopped the treatment but I am not ok. In the weeks following, I continued to have daily panic attacks, horrible vivid nightmares, fatigue, weakness, and emotional instability. My body has not been able to relax since this all started. I am always in a state of panic where my body is locked up and everything feels tight or I feel weakness and faint. I have to take tranquilizers everywhere I go still because I’m scarred that I will have an attack. Although I’m still having side effects from the Accutane ,the sinus pressure, constipation, dark urine, dry eyes and pain, dry lips have all returned to normal since I've stopped taking the Accutane.

I also think it’s important to note that I was diagnosed as having migraines with aura (seeing flashes of light or a blind spot in your line of vision before the pain starts) before I started taking Accutane and the occurrence of auras has increased dramatically (my head doesn't hurt but I see small flashes of light for about 3-4 seconds once or twice a week). I’m really hoping that these symptoms won’t be permanent.

The worst part is the lack of support to treat these symptoms. My family doctor didn't know anything about Accutane until I suggested that maybe all these symptoms are from the drug. She said maybe and went on to do a Google search about the drug. My dermatologist was also fairly useless and besides telling me to stop taking the Accutane, has not suggested anything I should do or anyone I should see to help treat these symptoms or find out what happened to me. Sorry for the long post but I hope that anyone considering taking Accutane are aware of the side effects and that they are not as rare as you think.

I’m not against Accutane. I think it could do wonders for people with severe acne but it should not be a drug that’s taken lightly. People need to realize that some of the more severe effects occur more frequently than you think and happen to people that are generally healthy. Personally if I knew all this was going to happen to me I could have lived with the acne. I wish I could just go back to normal and have my life and body back.

Roaccutan - esperienze negative

vorrei condividere con voi alcune esperienze negative di persone che hanno curato il proprio acne con Roaccutan. 
Da quanto si evince, in alcuni casi ha causato dei danni gravi ed irreversibili, allora io mi chiedo, vale davvero la pena di rischiare così per la propria pelle?
Ragazzi io ve lo sconsiglio. Cercate metodi alternativi per guarire dal vostro acne. Credetemi, capisco che in certi casi la disperazione spinge ad optare per metodi forti, ma non è sempre vero che una cosa forte è una cosa giusta.
Ho cercato di raccogliere un po' in Internet alcune esperienze negative per poterle dare a voi.
Mi piacerebbe ricevere da voi qualche commento a tal proposito.
Se avete avuto esperienze negative, mi farebbe piacere sentirle, vorrei fossero condivise con chi le ha subite o con chi potrebbe un giorno subirle qualora decidesse di ricorrere a questo farmaco.
In collegamento con il Roaccutan sono state segnalate 2000 depressioni, un migliaio fra suicidi tentati e riusciti - e in tutto oltre 6000 disturbi a carico della psiche. Questi dati sono contenuti nella banca-dati degli effetti collaterali dell'Organizzazione mondiale della Sanità e riguardano il Roaccutan, prodotto dalla multinazionale svizzera Roche. Secondo le autorità americane, questi numeri andrebbero moltiplicati per cento – perché solo l’1 per cento degli effetti collaterali provocati dai farmaci viene segnalato. Stando ai dati, il Roaccutan starebbe provocando una strage. Il farmaco è stato per due decenni un campione di incassi per Roche – il brevetto è scaduto nel 2002 ed oggi sono presenti sul mercato numerosi generici. Ad assumerlo sono soprattutto adolescenti, mentre dall'inchiesta di Falò emerge come addirittura medici e autorità sanitarie siano disinformate sui rischi del Roaccutan. Eppure sono ben 152 gli effetti collaterali identificati. A Falò i risultati sconvolgenti degli ultimi studi sul farmaco ma soprattutto la denuncia di un caso di suicidio, verificatosi durante la sperimentazione del Roaccutan, e sempre tenuto nascosto dalla Roche. Dal Ticino agli Stati Uniti, dall'Irlanda alla Svizzera parlano le vittime, gli avvocati, gli scienziati e Swissmedic. Grande assente, la multinazionale svizzera: su Roaccutan, Roche non rilascia dichiarazioni. 

Il Roaccutan mi ha rovinato

Ciao a tutti...

Io presi il Roaccutan a 17 anni; nelle avvertenze si parlava di depressione ed altri possibili effetti collaterali, ma si garantiva che sarebbero scomparsi dopo la fine della cura.

Fino a quel momento ero stato il ragazzo più felice del mondo, ero aperto ed espansivo, eppure mi venne la depressione e gli attacchi di panico.

Tutto questo cominciò un mese dopo la fine della terapia; la depressione si prolungò per tre anni, invece gli attacchi di panico continuano ancora adesso, che ho 29 anni. Poi cinque anni fa, per caso, ho scoperto dei forum in inglese, dove scrivevano persone che avevano avuto problemi con il Roaccutan: in gran parte erano cominciati dopo la fine della terapia, e si erano prolungati per qualche anno.

Oggi alcuni scienziati sostengono che molti degli affetti collaterali del Roaccutan cominciano proprio dopo la fine dell’ assunzione. Non sto dicendo che questo farmaco dà problemi a tutti, perché stiamo comunque parlando di una minoranza di pazienti; eppure è una minoranza molto più grande di quello che può sembrare a prima vista, perché viene segnalato solo l’ 1% degli effetti collaterali ( questa è una statistica che riguarda tutti i farmaci):pensate a quante persone ci possono essere che hanno avuto problemi con il Roaccutan ma non navigano su internet, oppure ci navigano ma si fidano del dermatologo e del foglietto illustrativo, e non si mettono a cercare “Roaccutan” su google; Quindi molta gente non collega questi effetti collaterali con il farmaco.

Per togliere l’ acne non vale la pena prendersi questi rischi; non stiamo parlando di un farmaco per il cancro o per il cuore.

E poi ci sono troppe cose che puzzano…alle ragazze fanno firmare quel foglio in cui vengono informate del rischio di malformazioni per il feto(in quel caso la donna si impegna a prendere gli anticoncezionali), ma perché fanno firmare un foglio anche ai ragazzi? Io non ricordo bene cosa firmai, ma dovevo farlo.

Si è mai vista una società farmaceutica che richiede la firma per l’ assunzione di un farmaco? E’ chiarissimo che qualcosa non quadra, ed hanno sicuramente la coscienza sporca…secondo me lo fanno per paura dei processi.

Esperienza Roaccutan
Ciao,sono una signora di 51 anni, ti scrivo per riportarti la mia esperienza a lungo termine. Ho usato il Roaccutan nel lontanissimo 1984, allora ero in cura da un dermatologo di Bellinzona Svizzera. Il farmaco era arrivato credo l'anno prima.
Il foglietto illustrativo non riportava innumerevoli effetti collaterali che si sono invece rilevati con il tempo. Maledetto il giorno che ho iniziato quella cura. Ancora oggi a distanza di 27 anni ne riporto le conseguenze. Al termine dei 5 mesi di cura ho iniziato a perdere capelli, una caduta diffusa e diradante, mai cessata,i bulbi piliferi sono stati danneggiati in modo permanente, per farti capire sono atrofizzati, la maggior parte morti, quindi non producono più capelli, mentre quelli rimasti sono deboli e producono capelli fini e senza vitalità'. Dal punto di vista psicologico poi anche peggio, ansia e attacchi di panico non mi hanno mai abbandonata. Io mi sono iscritta all'Associazione vittime del Roaccutan e generico. A.V.R.G. E' una associazione francese. Io purtroppo non parlo il francese e non sono nemmeno brava con l'uso del PC.
Ma voi giovani dovete sapere che non e' un farmaco, ma un veleno. Mi assumo tutte le responsabilità' di quello che dico e ho anche contattato la ditta Roche produttrice di questo veleno, naturalmente senza esito.

Al bando il roaccutan!!!!
Pochi sanno e pochissimi medici dicono e mettono al corrente i propri pazienti degli effetti devastanti del farmaco Roaccutan. 
Io presi il farmaco per tre mesi 5 anni fa. Faccio una premessa: i benefici del farmaco durarono solo per il tempo di assunzione...Da lì a pochi mesi la mia acne tornò allo stesso stato d'origine. Le manifestazioni più importanti dal punto di vista psicologico furono molte e il farmaco ha cambiato moltissimo il mio umore: ero nervosa, apatica ecc. (tra parentesi sono sempre tata iperattiva)...Torniamo agli effetti del farmaco ma NEL LUNGO PERIODO: tutti sanno che il rischio maggiore è dovuto alla presenza di una gravidanza, pochi che, anche sospeso, il farmaco, e a distanza di anni dalla sospensione, dà effetti irreversibili nella donna come la modificazione del DNA delle ovaie...Tanto è vero che quanti vogliono assumere il farmaco, al di là della consapevolezza dei rischi in gravidanza e dell'accettazione per iscritto, debbono pure firmare una LIBERATORIA al medico che prescrive il farmaco nonché alla DITTA PRODUTTRICE CIOÈ LA ROCHE...Ricordate o cercate su internet l'elenco delle vittime della casa Roche!!!! E ora, vogliono testare su di noi questo farmaco... NE VALE LA PENA???? Parla una persona piena di acne!!!! Me la tengo piuttosto che rovinarmi la vita con altri problemi ancora!!!! Non hanno forse figli anche i loro titolari, i loro chimici, i loro commerciali? Io non potrei mai creare con le mie stesse mani qualcosa che possa intaccare l’essere umano.
Hanno forse disinteresse verso gli altri per creare prodotti nocivi? Oppure la smania di successo e denaro è più importante del creare qualcosa che possa essere realmente utile senza essere tossico?
Molti di questi ingredienti, che non dovrebbero neanche essere permessi, hanno un costo irrisorio, forse è anche per questo che qualcuno decide di farne uso.
Per fortuna ci sono anche aziende serie.
Una di queste è doBrasil. Io ho scoperto questa azienda per caso, andando a comprare un integratore su un negozio Isola Verde Erboristerie. Qui mi sono stati regalati alcuni campioncini di creme e detergenti doBrasil, il tutto poi mi è stato consegnato su una bella borsetta nera doBrasil.
Mi sono incuriosita leggendo i claim sui campioncini e sulla borsetta stessa. Appena tornata a casa ho voluto provare tutto. E mi sono innamorata di doBrasil.
I prodotti hanno una bella consistenza addosso, non senti di averli, ma sai di averli perché ti accorgi da subito che qualcosa sta migliorando in te.
Sarà merito degli ingredienti bioattivi contenuti. Pur essendo idratante, non lascia la pella unta e grassa – è per questo che è perfetta per me.
Poi nei momenti di maggiore secchezza, mi faccio un bel “bagno idratante” con la maschera purificante.

Che dire, io non ne posso più fare a meno di questi prodotti!!
Sonò così contenta che uso anche lo shampoo brasil nut per capelli delicati.
Anche i prodotti per la detersione del corpo e le creme stesse hanno dei profumi inebrianti.

Accutane: do you want to run the risk?

I would like to share your opinions about Accutane.
I think everybody who suffers from acne, knows this medicine and its dangerous side effects.
Personally I have never opted to use it, because even my doctor suggested me not to take Accutane, she instructed me the risks and sincerely said it was better not to use it.
Her son suffered from acne, he was cured with Accutane and he recovered.
Even thought my acne was serious and persistent (it was withstanding antibiotics, laser treatments and other strong cures – for sure lighter than Isotretinoin), she suggested me no to follow this dangerous cure, even more so because I am a woman. Isotretinoin, contraindicated while pregnancy and breastfeeding, is discouraged during fecund age if strong pregnancy preventions are not satisfied. Women have to follow a rigid contraceptive program during the entire consumption period, for one month before and one month later than the consumption of the medicine. During the treatment with Accutane, it is necessary to do periodically pregnancy tests in order to avoid every possible risk. This actions are essential because Accutane can cause abortions and foetus deformities.
Moreover, after the consumption of Isotretinoin, more cases of depression, anxiety, aggressiveness, humour disorders, psychotic manifestations and sometimes suicide thinks, suicide attempts and suicides have been registered.
During the treatment it is recommended to be under strict control of the doctor, to notice each anomaly or side effect due to Isotretinoin.
All these facts convinced me not to use Accutane, and I am really happy for my decision, also because I could recover from my acne thanks to Zineryt, a medicine which did not cause me noise and did not worried me with particular side effects.

How should I take Accutane?

Avoid exposure to sunlight or artificial UV rays (sunlamps or tanning beds). Accutane can make your skin more sensitive to sunlight and sunburn may result. Accutane may impair your vision, especially at night. Be careful if you drive or do anything that requires you to see clearly.

Accutane side effects

Get emergency medical help if you have any of these signs of an allergic reaction to Accutane: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using Accutane and call your doctor at once if you have any of these serious side effects:

Roaccutan & Co: vale la pena correre certi rischi?

Buongiorno ragazzi,

vorrei aprire con voi un dibattito su Roaccutan.

Penso che chiunque soffra di acne, sia a conoscenza di questo farmaco e dei pesanti effetti collaterali che questo può causare. Io personalmente non mi sono mai avvicinata all’idea di poterlo assumere, anche sotto suggerimento del mio medico curante, il quale mi ha messo in guardia sui rischi e mi ha sinceramente indicato di evitarlo.

Suo figlio soffriva di acne e ed stato in cura con l’Isotretinoina ed è guarito.

Tuttavia, nonostante il mio acne fosse grave e resistente a qualsiasi trattamento forte, pur sempre meno forte di questo farmaco (antibiotici, laser), mi è stato suggerito di non ricorrere a questa cura pericolosa, a maggior ragione perché donna. L’Isotretinoina, oltre ad essere controindicata in gravidanza e in allattamento, è sconsigliata in età fertile se non vengono soddisfatte alcune condizioni di prevenzione di gravidanza. Occorre seguire un rigido programma anticoncezionale durante tutto il periodo di assunzione del farmaco, e per un mese prima e un mese dopo la sua assunzione. Durante il trattamento è necessario eseguire periodicamente test di gravidanza per poter evitare qualsiasi rischio. Tutto questo procedimento è indispensabile, in quanto il farmaco può provocare aborti e malformazioni al feto.

Inoltre, in seguito all’assunzione di Isotretinoina per via orale, sono stati segnalati episodi di depressione, aggravamento della depressione, ansia, aggressività, alterazioni dell’umore, sintomi psicotici e, molto raramente, idee suicide, tentativi di suicidio e suicidio.

Durante il trattamento inoltre, è consigliato uno stretto controllo da parte del medico curante, per poter prendere visione di qualsiasi anomalia o effetto indesiderato derivato dall’assunzione del farmaco.

Tutto questo mi ha convinto a non assumere Roaccutan, e sinceramente sono rimasta molto contenta della mia decisione, anche perché ho potuto comunque guarire dall’acne grazie a Zineryt, un farmaco che non mi ha causato alcun disturbo e che non mi ha preoccupato con alcuna controindizione importante.

Personalmente non sono a favore di Roaccutan, e voi cosa ne pensate?

ACNE: IL ROACCUTAN EFFICACE MA ASSOCIATO A GRAVI EFFETTI INDESIDERATI. ADDIRITTURA RILEVATO RISCHIO DEPRESSIONE

Acne: il Roaccutan efficace ma associato a gravi effetti indesiderati. Addirittura rilevato rischio depressione sino al suicidio fino ad un anno dopo la fine del trattamento. Non sono pochi, i giovani che hanno problemi di acne, spesso in forma acuta. Per il trattamento di questa patologia, vi sono numerosi farmaci, ma l’Isotretinoina (acido-cis-13-retinoico), in Italia commercializzata con il nome di Roaccutan (Accutane negli Stati Uniti), è quello attualmente più potente, oggi disponibile, per il trattamento della forma grave di acne. Un ciclo di trattamento con Isotretinoina della durata di 5 mesi permette una remissione prolungata dell’acne in più dell’85% dei pazienti. Alcuni pazienti possono richiedere più di 1 ciclo di trattamento. Il farmaco in questione, secondo quanto è dato apprendere da alcune ricerche, presenta alcune controindicazioni che è sempre opportuno conoscere prima d’iniziare il trattamento. Tra i rischi, è noto che può causare aborto e gravi difetti al nascituro. Inoltre, i pazienti che assumono il farmaco possono incorrere in gravi problemi che interessano un certo numero di organi, tra cui il fegato, l’intestino, gli occhi, il naso e l’apparato scheletrico. Poiché l’Isotretinoina è un farmaco ad alto rischio il suo impiego dovrebbe essere riservato ai casi di acne cistica recalcitrante grave. Questo tipo di acne è resistente al trattamento standard, tra cui gli antibiotici, ed è caratterizzata da molte lesioni infiammatorie nodulari e cistiche piene di pus. Queste lesioni possono causare dolore, cicatrici permanenti ed effetti psicologici negativi. L’FDA (Food and Drug Administration), l’agenzia federale statunitense che vigila sui farmaci, ha approvato l’Isotretinoina nel 1982 e, da allora, quasi 5 milioni di americani e 12 milioni nel mondo sono state trattati con Isotretinoina, secondo i dati forniti dalla casa produttrice Roche. La metà delle persone che assumono il farmaco sono di sesso femminile, la maggior parte delle quali è in età fertile ( 15-44 anni ). Il rischio di difetti alla nascita tra le donne che assumono l’Isotretinoina è estremamente alto. Questi difetti comprendono idrocefalia e microcefalia, difetti cardiaci, deformità facciali, come labbra leporine e orecchie mancanti, e ritardo mentale. Dati di letteratura indicano che il 5-35% dei feti esposti all’Isotretinoina andranno incontro ad una malformazione. Nonostante nella scheda tecnica del prodotto fosse indicata la tossicità fetale, diverse segnalazioni di gravi difetti alla nascita sono giunte subito dopo la commercializzazione del farmaco. Allo scopo di ridurre l’uso del farmaco in donne fecondate, la società produttrice Roche, nel 1988, diede vita al Programma PPP (Pregnacy Prevention Program), in italiano Programma di Prevenzione della Gravidanza, per educare ulteriormente le donne ed i medici ad evitare i casi di tossicità fetale. Alle donne in età fertile che assumevano Isotretinoina veniva anche consigliato di seguire un programma anticoncezionale un mese prima, durante, ed un mese dopo il trattamento. Tuttavia, il Programma PPP non è riuscito ad eliminare del tutto le gravidanze in donne che stavano assumendo l’Isotretinoina. Nel 2005, è stato approvato il Programma iPLEDGE con maggiori restrizioni. iPLEDGE permette di ricevere una confezione di Isotretinoina solo se le donne vengono registrate in questo programma elettronico. Al di la di questi aspetti più noti e pertanto oggetto di maggiore attenzione da parte dei soggetti, in particolare donne che l’assumono, e dei sanitari che la prescrivono, vi sono altre avvertenze sulle quali lo “Sportello dei Diritti”, vuole porre l’accento per ricordare che questo farmaco deve essere prescritto solamente da o sotto la supervisione di medici che abbiano esperienza nell’uso dei retinoidi sistemici per il trattamento dell’acne grave e che comprendano pienamente il rischio del trattamento con isotretinoina e la necessità di monitoraggio. Infatti, in alcuni pazienti, l’assunzione dell’Isotretinoina è associata a gravi disturbi psichiatrici. Nonostante che la depressione fosse inserita nella scheda tecnica di prodotto, l’FDA nel 1998 ha ritenuto opportuno ri-sottolineare il pericolo di gravi disturbi psichiatrici nel corso del trattamento con Isotretinoina. Nella scheda tecnica è stata inserita l’avvertenza che l’assunzione di Isotretinoina può produrre depressione e psicosi, e che in rari casi il paziente può andare incontro ad ideazione suicidaria, tentativi di suicidio e suicidio. Tuttavia la relazione tra Isotretinoina e depressione non è stata scientificamente dimostrata. È ovvio, infatti, che alcuni pazienti, all’atto di assumere l’Isotretinoina, potrebbero già soffrire di depressione. Come detto, il farmaco in questione viene utilizzano per la cura di forme severe di acne nodulare recalcitrante. Tuttavia, la pubblicazione di alcuni case-report e la segnalazione spontanea di reazioni avverse hanno suggerito un'associazione tra isotretinoina, depressione e comportamenti suicidari, anche se gli studi effettuati hanno fornito risultati contrastanti. Quindi, in tal senso, è opportuno riportare un'ampia sintesi di una ricerca pubblicata sul British Medical Journal. In questo studio sono stati presi in considerazione pazienti trattati con isotretinoina per l'acne grave tra il 1980 e il 1990 e di età compresa tra 15 e 49 anni al momento della prima prescrizione. I dati sono stati collegati a quelli dei registri dei ricoveri ospedalieri e di mortalità (1980-2001). Nel corso dello studio è stato effettuato, in 3 diverse finestre temporali (prima, durante e dopo il trattamento), un confronto tra il tasso di tentati suicidi nel gruppo preso in esame e quello nella popolazione generale utilizzando il registro nazionale dei ricoveri ospedalieri. I dati sono stati stratificati per sesso, 5 fasce di età e per anno di calendario allo scopo di calcolare il numero atteso di eventi nella popolazione in studio in ciascuna finestra temporale. L'outcome principale era rappresentato dal tasso di incidenza standardizzato (calcolato dal rapporto tra numero di tentati suicidi osservati e numero di eventi attesi standardizzati per sesso, età ed anno) calcolato a partire dai 3 anni precedenti, durante e fino a 15 anni dopo la fine del trattamento. Sono stati esaminati 5.756 pazienti di cui il 63% di sesso maschile. L'età media per gli uomini era di 22,3 anni, mentre per le donne era 27,1 anni. La durata media del trattamento era di 6 mesi per gli uomini (durata totale 1.819 anni-persona) e 6,1 mesi per le donne (durata totale 1.091 anni-persona). Oltre il 90% dei pazienti ha ricevuto un solo ciclo di trattamento. Tra il 1980 e il 2001, il 2,2% dei pazienti (1,9% degli uomini; 2,7% delle donne) è stato ricoverato almeno una volta per tentato suicidio, per un totale di 210 ricoveri registrati (1,6 per persona). Il tasso di incidenza standardizzato, ricavato confrontando il gruppo in esame con la popolazione generale, è aumentato gradualmente per il primo tentativo di suicidio da 0,89 (IC 95% 0,54-1,37) nei 3 anni prima del trattamento a 1,36 (0,65-2,50) nell'anno precedente il trattamento del primo tentato suicidio e da 0,99 (0,65-1,44) nei 3 anni prima del trattamento a 1,57 (0,86-2,63) nell'anno precedente il trattamento per tutti i tentativi. Il rischio è risultato maggiore nei 6 mesi successivi all'inizio del trattamento: 1,93 (1,08-3,18) per il primo tentativo e 1,78 (1,04-2,85) per tutti i tentativi. Infine, è stata osservata una diminuzione del tasso di incidenza standardizzato nei 3 anni successivi al trattamento: 0,97(0,64-1,40) per i primi tentativi e 1,04 (0,74-1,43) per tutti i tentativi. Non è stata rilevata alcuna differenza statisticamente significativa nella dose giornaliera prescritta o nella durata del trattamento tra le persone che hanno tentato il suicidio e quelle esaminate in totale. Tuttavia, 14 pazienti (donne) che avevano tentato il suicidio dopo il trattamento avevano ricevuto 2 o più cicli di trattamento (24%; 13-35%) rispetto alle donne mai ricoverate per tentato suicidio (9%, 8-10%).
